What is the Active Referral Scheme?

A selection of physical activity programmes for people with medical conditions, whose health would benefit from being more active. Each programme helps people to be active in a safe environment and manage the symptoms of their condition.

The following programmes are available under the Active Referral Scheme:

  • Exercise Referral
  • Cardiopulmonary Rehabilitation
  • Back Pain Programme
  • Steady Steps
  • Fit4Surgery Cancer Prehabilitation - Patients listed for major cancer surgery. Requires specialist hospital referral.

Activities will differ depending on the programme and location but typically include:

  • Fitness/gym based personalised exercise programmes
  • Swimming pool-based activities including aqua aerobics
  • Low impact group exercise classes such as Tai Chi, seated exercise or circuits
  • Group health/guided walks
  • Green gym projects such as community allotments and conservation projects
  • Virtual physical activity sessions where appropriate

Exercise Referral

Exercise Referral is 12 week programme of personalised physical activity support for people with specific health conditions.

Qualified exercise professionals create a safe and effective physical activity programme tailored to your needs and goals.

Exercise Referral is available to people aged 16 years and over with an existing health condition that could be improved by physical activity. Your programme will be adapted by our experienced instructors to best support the management of your health condition.

You'll need to ask your GP to be referred. Full eligibility details can be found in the document below. If you are eligible you'll be invited to a welcome meeting. This will typically take place within a local leisure facility on a 1-1 basis with an exercise referral instructor. You'll be invited to discuss your goals and activity preferences to help create your own tailored 12 week programme.

  • Swimming pool-based activities including aqua aerobics
  • Low impact group exercise classes such as Tai Chi or seated exercises
  • Guided Health Walks
  • Virtual physical activity sessions where appropriate

Regular reviews will take place to adapt your programme if required throughout the 12 weeks. At the end of the programme you'll be invited to attend an exit consultation. Your progress will be reviewed and opportunities to continue being active will be discussed.

Cardiopulmonary Rehabilitation

Cardiopulmonary Rehabilitation (Phase IV) is a 12-16 week group based exercise programme for people who have recently experienced a cardiac event e.g. heart attack.

This community based programme is delivered in a safe and controlled environment by fully qualified and experienced cardiac rehabilitation exercise specialists.

The programme is suitable for people who have successfully completed phase III cardiac rehabilitation in Hospital within the last 6 months. Some local schemes may allow people to enter the programme without completing Phase III if they have a previous history of heart disease.

This programme is specially designed as a continuation of supervised physical activity following on from exercise classes delivered in hospital. This programme will support your full rehabilitation following your cardiac event. It will give you the freedom to fully enjoy life again, boost your confidence and reduce your risk of having another cardiac event.

Activities will vary across areas and venues. Most cardiac rehabilitation Phase IV programmes offer a low impact circuit class supervised by a fully qualified cardiac rehabilitation exercise instructor. Where available guided health walks and virtual exercise classes may also be available.

Regular reviews will take place to adapt your programme if required throughout the 12-16 weeks. At the end of the programme you'll be invited to attend an exit consultation. Your progress will be reviewed and opportunities to continue being active will be discussed.
